Medical Cannabis Reciprocity

East Honolulu residents should not assume their Hawaiʻi 329 card will provide the same rights when traveling outside the state. Medical cannabis programs are governed separately by each jurisdiction, and recognition of visiting patients can differ substantially.

Hawaiʻi itself requires qualifying visitors to obtain temporary registration instead of simply presenting an out-of-state medical cannabis card at a local dispensary.

What Does Medical Cannabis Reciprocity Mean?

Reciprocity generally refers to one jurisdiction recognizing, to some extent, the medical cannabis authorization issued by another. Depending on local law, visiting patients may receive temporary registration, permission to purchase from dispensaries, possession protections, or no privileges at all.

Because there is no uniform nationwide system, East Honolulu patients should review the official medical cannabis requirements of their destination before relying on Hawaiʻi registration.

Can Out-of-State Patients Use Their Medical Cannabis Cards in Hawaiʻi?

Possessing a medical cannabis card issued elsewhere does not, by itself, authorize dispensary purchases in Hawaiʻi. Certain visitors from U.S. states or territories may apply for temporary Hawaiʻi registration when they hold a valid medical cannabis card from their home jurisdiction and meet Hawaiʻi’s qualifying-condition requirements.

An approved visitor receives a temporary digital 329 card that may remain valid for up to 60 days. A qualifying out-of-state patient may obtain another temporary registration once during the same calendar year, allowing no more than 120 days of registration annually. The out-of-state registration application currently costs $49.50.

Can a Hawaiʻi 329 Card Provide East Honolulu Patients With Access Elsewhere?

A valid Hawaiʻi registration may receive some recognition in jurisdictions that accommodate visiting medical cannabis patients, but a 329 card does not guarantee purchasing or possession rights outside Hawaiʻi.

Some destinations require a separate visitor application, others may provide only limited protections, and certain jurisdictions do not recognize Hawaiʻi medical cannabis registration at all.

Which States Accept Hawaiʻi 329 Cards in 2026?

Hawaiʻi DOH does not maintain a definitive list guaranteeing acceptance of 329 cards in other states. Since visitor-access and reciprocity laws are determined by each destination and may change, East Honolulu patients should check directly with the appropriate government medical cannabis program before traveling.

Patients should also avoid taking cannabis from Hawaiʻi to another jurisdiction. Travel involving cannabis can cross federal jurisdiction, where different laws apply.
